Community Pharmacy Derbyshire have recently received resignation from Lindsey Fairbrother, who was an Independent member on the committee.
Independent pharmacies in Derbyshire will be receiving this information and nomination form through the post but it is also below.
If you would like to be a part of the Community Pharmacy Derbyshire committee then you should put forward a nomination by completing the below nomination form and returning to Alison Ellis at the following email address: info@cpderbyshire.org.uk by 5pm on Wednesday 6th May 2026.
If we receive more than one nomination there will be an election and voting papers will be distributed to eligible contractors. Accompanying the voting paper will be the information taken from the shaded boxes on the nomination paper to provide voters with information on each of the candidates.
