Request for authorisation to continue managed repeats form (MOL)
Please see below the Request for Authorisation to Continue Managed Repeats form from the Medicines Order Line.
The forms are only to be used if a pharmacy is ordering through the MOL on behalf of a patient because the patient is unable to order via the MOL themselves.
Pharmacies should complete the request to continue a managed repeat form and send these onto practices for them to update the patient record for next time their medication is to be ordered.
MDS patients do not need one of these forms completing where pharmacies order direct through the surgery and not through the MOL.
If a form has already been completed and returned to the surgery then there is no need to complete another. Only complete for any new patients that you need to order for on their behalf
